ALK-Rewards: Terms and Conditions A. Overview of ALK-Rewards a. Becoming a member: To join the ALK-Rewards program, fill out the enrollment form available at www.alk-abello.us or by contacting local ALK Allergy Consultant. Once survey is completed, account will be automatically enrolled in both the Fight the Cause campaign and ALK-Rewards. Participation in ALK-Rewards is subject to the terms and conditions, rules, regulations, policies and procedures that ALK-Abelló, Inc., may adopt from time to time. ALK-Abelló, Inc. may amend the Terms and Conditions and Program Rules at any time without notice. ALK-Abelló, Inc. has the sole discretion to interpret and apply the Program Rules. b. Qualifying for Medallion Levels: Accounts qualify for medallion levels based on the sum of Loyalty Points and Activity Points. Upon joining ALK-Rewards, account is placed in a medallion level based on certain account activities and sales from prior calendar year. Once medallion level is achieved, it is maintained as long as account’s sales grow by 10% over prior year’s sales OR the number of product categories does not decrease. An account is eligible to reach the next level as long as they meet the above condition to maintain current level AND exceed minimum level of points for next level. To advance to Platinum, account must purchase a minimum of 12 eligible categories for Bulk customers / 4 eligible categories for Treatment Set customers, in addition to meeting the above conditions. To advance to Platinum Elite, account must purchase all eligible categories from ALK-Abelló, Inc., in addition to meeting the above conditions. c. Earning points: Points are earned in two ways.
1. Activity Points are earned by participating in activities with ALK-Abelló. Each eligible activity has a defined point value.
2. Loyalty points are earned through purchases in the eligible product categories based on the type of account. For an account to earn points for purchasing within a category, sales must exceed $250 per calendar year for that category. For new product group sales, credit is given in the month after the account meets the $250 sales requirement. Bulk customers purchase from 14 eligible categories and receive 5,000 points per eligible category purchased. Certain restrictions may apply and will be addressed by individual account:
1. Cat
2. Grasses
3. Molds
4. Weeds
5. Trees
6. Mite
7. Venom
8. Histamine
9. Syringes
10. Ancillary: includes sterile empty vials, diluents, and other items such as trays and labels.
11. Skin Testing: includes AccuSet™, DuoTip-Test II®, and Multi-Test II®.
12. Diagnostics: includes all 5ml dropper or 5ml intradermal prepared vials, regardless of extract type- i.e. cat, mite, grass in droppers are all diagnostic products.
13. Other & Center-Al®: includes all Center-Al® Alum adsorbed products as well as miscellaneous pollens, bulk foods (10ml size or greater), epidermals, miscellaneous inhalants, house dust, insects and special mixes.
14. PRE-PEN
Note: Bulk customers will also receive 5,000 points for purchases of Treatment Sets over $250, but purchase of this category will not count toward the number of categories required to reach Platinum and Platinum Elite status.
 
Treatment set customers typically purchase from 7 eligible categories and receive 10,000 points per eligible category purchased. A treatment set customer is classified as an account whose sales of treatment sets exceed sales of all other bulk categories combined and is greater than $250:
1. Treatment Sets
2. Venom
3. Histamine
4. Syringes
5. Skin Testing: includes AccuSet™, DuoTip-Test II®, and Multi-Test II®.
6. Diagnostics: includes all 5ml dropper or 5ml intradermal prepared vials, regardless of extract type- i.e. cat, mite, grass droppers are all diagnostic products.
7. PRE-PEN
Note: Treatment Set customers will also receive 10,000 points for purchases outside of eligible categories over $250, but purchase of these categories will not count toward the number of categories required to reach Platinum and Platinum Elite status. d. Account Activity Statement: Account statements, detailing point balance and available rewards will be sent out quarterly.
 
For updates more often, visit www.alk-abello.us. Loyalty and most Activity points will be updated on a monthly basis. All exceptions will be updated on a quarterly or yearly basis. e. Redeem Rewards: Rewards are redeemed by request through your ALK Allergy Consultant or Customer Service through the website. Rewards are subject to resource availability and scheduling constraints. Each reward can only be redeemed a specified number of times.
 
Availability is subject to change by ALK-Abelló, Inc. at any time without notice.
 
Customized marketing materials and referral development availability is based on geographic region and other practices’ prior use. The customized mailing to area physicians is limited to a list of 50 physicians in the account’s area.
 
All rewards must be redeemed within the calendar year they are earned. All rewards expire at the end of the calendar year, and are re-awarded at the beginning of the calendar year after reassessment of account eligibility based on requirements to maintain medallion level.
 
It is possible for account to move down in levels, as well as up. f. Duplicate Accounts: Only one (1) ALK-Rewards account is allowed per ALK-Abelló, Inc., account number. Multiple doctors in a single practice will all participate in ALK-Rewards as they do with their ALK-Abelló account (e.g., if five doctors order through one ALK-Abelló account number, they will share one ALK-Rewards account). ALK-Rewards accounts will use the same ALK-Abelló account number used for ordering. Duplicate accounts are not eligible to earn points or redeem rewards and will be cancelled.
 
Modification and Termination of the Program: a. ALK-Abelló, Inc. may modify any of the terms and conditions governing the Program - including, but not limited to, the methods through which points can be earned, rewards offered, medallion level requirements, minimum purchase requirements per category and the conditions under which points may be forfeited - at any time, with or without notice, even though these changes may affect an accounts ability to use the points that the account has already earned. b. The Program has no predetermined termination date and may continue until such time as ALK-Abelló, Inc., decides to terminate the Program. ALK-Abelló, Inc. may terminate the program at any time, with notice via mail to the account’s address currently on file with ALK-Abelló, Inc. Members will have six (6) months from the date Program termination is announced to use rewards. Any rewards that are remaining in a Member's account at time of termination will be forfeited and no compensation will be provided, no matter the extent of member participation in the program. c. A Member's continued participation in the Program constitutes the Member's acceptance of any changes to these Terms and Conditions.
 
Members are responsible for remaining knowledgeable as to any changes that ALK-Abelló, Inc., may make to these Terms and Conditions. The most current version of these Terms and Conditions will be available on the Website and will supersede all previous versions of these Terms and Conditions. S273-v4
